Up to 40% of patients with Stage II-III renal carcinoma eventually suffer a recurrence despite radical nephrectomy (RN). We present the case of a 40-year-old male with a diagnosis of clear cell carcinoma, who underwent RN with regional lymphadenectomy. He was on follow-up for 3 years with no evidence of metastasis. He came for consultation 19 years later, referring to pain in the left hypochondrium. Abdominal multislice CT scan showed a tumor in the left renal fossa. On positron emission tomography (PET/CT), a retroperitoneal tumor with active disease was confirmed, so resection was performed. This case emphasizes the need for long-term rigorous follow-up.
