Abstract

Headache is one of the most common reasons for outpatient consultations in neurology. Therefore, distinguishing high-risk (secondary) headaches can be challenging, especially in patients with pre-existing headaches. We report the case of a 56-year-old female with a history of migraine, in whom extracranial aneurysms at the bifurcation of both carotid arteries were incidentally discovered. Surgical intervention of the left aneurysmal segment was performed, followed by an end-to-end anastomosis. The coexistence of migraine and bilateral carotid artery aneurysms represents a rare clinical presentation. This case highlights the importance of a thorough evaluation in patients with difficult-to-manage headaches, as well as the accurate diagnosis and treatment of potentially dangerous conditions. Additionally, it underscores the value of interdisciplinary collaboration to achieve comprehensive patient care.
