Non-surgical endodontic retreatment for the resolution of apical periodontitis: a literature review

Abstract

The objective of the study was to evaluate the efficacy of nonsurgical endodontic retreatment in resolving apical periodontitis, considering bacterial reduction, postoperative pain control, and periapical healing. A total of 35 articles selected from PubMed, Scopus, and Embase through October 2024 were reviewed, applying inclusion and exclusion criteria. The analyzed studies covered disinfection protocols (sodium hypochlorite, chlorhexidine, laser, and ozonated irrigants), obturation techniques, success and failure rates, as well as the elimination of resistant microorganisms such as Enterococcus faecalis and Fusobacterium nucleatum. The findings indicate that the use of advanced technologies, such as the 980-nm diode laser and ozonated irrigants, enhances disinfection and promotes periapical repair. Likewise, ultrasonic and manual activation of irrigants proved effective in reducing bacterial load and alleviating postoperative pain; however, the heterogeneity of the protocols highlights the need to establish standardized, evidence-based guidelines to optimize outcomes. In conclusion, nonsurgical endodontic retreatment is an effective alternative for the management of apical periodontitis, especially when modern disinfection methods are used; however, further research is needed to consolidate clinical practice.
