Cuidados de enfermería a un adolescente con homofobia internalizada desde el abordaje del modelo conductual de Johnson

La homofobia internalizada en la adolescencia constituye un fenómeno que impacta negativamente en la salud mental y favorece alteraciones conductuales significativas. Desde el Modelo del Sistema Conductual de Dorothy E. Johnson, la enfermería actúa como un agente regulador externo orientado a restablecer el equilibrio conductual del individuo. El objetivo fue analizar el cuidado de enfermería en un adolescente de 16 años con homofobia internalizada, síntomas ansioso-depresivos e ideación suicida previa, identificando alteraciones conductuales y formulando un plan de cuidados integral. La valoración evidenció alteraciones en los subsistemas de afiliación, dependencia, sexualidad y agresividad-protección, manifestadas a través de aislamiento social, lenguaje autodevaluativo, conflicto identitario y ansiedad persistente. Se priorizaron los diagnósticos de baja autoestima situacional, afrontamiento ineficaz y riesgo de violencia autodirigida. El plan de cuidados, estructurado con los sistemas NANDA-I, NOC y NIC, se ejecutó durante ocho meses mediante intervenciones semanales iniciales y seguimiento posterior; estas se centraron en el apoyo emocional, el fortalecimiento de la autoestima, la regulación afectiva y la elaboración de un plan de seguridad. La evaluación mostró una disminución del lenguaje autohomofóbico, mayor expresión emocional y el desarrollo de estrategias adaptativas, lo que evidenció una reorganización conductual acorde al modelo teórico.
