Objective: To determine the care provided by parents regarding post-vaccination reactions of their children under one year of age in the immunization service of a maternal and child health center in Zapallal (Lima, Peru). Material and methods: Quantitative, descriptive and cross-sectional study. The sample consisted of 60 participants with children under one year of age, selected by simple random sampling. The instrument was developed by the researchers, which contains 22 items and is composed of three parts: i) sociodemographic data, ii) parental care for local reactions, and iii) parental care for systemic reactions; it has also been validated through expert judgment and pilot test with KR = 0.712, demonstrating that the instrument is reliable. Results: Parents were characterized by being of secondary school level, cohabiting marital status, with an average of two children. In general, they reported providing adequate care in 58.33% of the post-vaccination reactions. When reviewing the post-vaccination care, it was found that 56.67 % were adequate for pain, 20 % for hardening and 41.67 % for redness. Regarding systemic reactions, care was adequate against identification of fever in 33.33 %, against fever in 91.67 %, against diarrhea in 60.00 %, in the presence of vomiting in 85.00 %, against loss of appetite in 23.33 %, and against irritability in 51.67 %. Conclusions: Adequate care prevailed in the majority of parents in the face of local and systemic post-vaccination reactions.
